Forms & Documents

To complete enrollment in our Bright Blocks Afterschool programs, please download, complete, and return the required forms below. All forms must be submitted before your child can participate in any activities.

How to Submit:
Completed forms can be uploaded during online registration, emailed to programs@brightblocksafterschool.com, or returned directly to the Bright Blocks Afterschool team at your child’s school.
